This setting of Nebo, zeml’a sotvoriňňa is from Father William Levkulic's Hymns of the Great Fast (on page 20).

Sources

The Slavonic version, with three verses, was printed on page128 of Father Stefan Papp's Duchovňi Pisňi (1969). It appears that Father Levkulic took and translated this version; I have not been able to find it in any other published source.
